Bring these with you:

Report to registration and show your referral letter; you will be directed to the right clinic.

For health facilities

Emergency or very urgent: call Casualty before the patient leaves — 0798 638 872 (24 hours).
Send a referral letter with the patient (findings, treatment given, reason for referral).
Optional: notify us in advance with the form below, so the receiving team knows the patient is coming.

The form asks for the patient's initials, age and sex only — never names or other identifying details.

For health facilities. Patient details: initials, age and sex only — the referral letter goes with the patient.
